Insurance

I do not accept health insurance of any kind, and I do not accept Medicare/Medicaid. I am considered an "Out-of-Network" provider. However, I am able to provide you statements for you to submit to your insurance company for reimbursement. This will depend on the individual plan, whether you have any ‘out of network’ benefits, and so I cannot guarantee any reimbursement/s.

It would be helpful to contact your insurance company beforehand, and ask the following questions:

1) "Do I have Out-of-Network coverage?" e.g. would you be able to submit my bills to your insurance for reimbursement

2) "What is my deductible for mental/behavioral health coverage?" e.g. how much must you spend out of pocket before insurance begins reimbursing

3) "What is my co-insurance?" e.g. what percentage of the fee will you NOT be reimbursed

4) "What is the allowed amount for the following codes?" e.g. these are medical billing codes, and most insurances have a limit to how much they will reimburse, based on the code. This amount may differ from my fees, thus a possible difference in reimbursement.

  • 99205 (initial psychiatric diagnostic evaluation)

  • 90833, 90836, and 90838 (add-on psychotherapy codes)

  • 99213, 99214, and 99215 (evaluation/management follow-up appointment codes) 

Payment

Payment will be due at the time of each visit, regardless of whether you decide to pursue reimbursement from your insurance provider. 
I accept cash, check, and all major credit cards as forms of payment.

Fees

  • Initial evaluation (may require additional follow-up visits in order to complete evaluation)

    • Adults- 60 - 90 minutes: $450 - $670

    • Children/Adolescents- 90 minutes: $670

  • Psychopharmacology (medication management)

    • 20-30 minutes: $ 180-$225

  • Psychotherapy +/- psychopharmacology

    • 45 minutes: $340

    • 60 minutes: $450

  • Phone calls for established patients, outside of scheduled appointments

    • $50 up to 15 min, and $50/15 minute increments thereafter

In compliance with the No Surprises Act issued January 1, 2022, it is important that all prospective and current patients review this fact sheet.

Cancellations and Missed Appointments

I require at least a 48 hour notice for any appointment cancellations. If less than 48 hour notice is provided, or if you miss a scheduled appointment without notice, you will be charged in full for the appointment. Emergency situations will be discussed on a case-by-case basis.
